Report Regarding Suspected Ethical Misconduct or Lack of Professionalism

The purpose of this form is to allow those not comfortable raising issues directly with another individual, or those who have attempted to voice concerns without a productive outcome, to report concerns. In most circumstances, it is expected the first course of action will be to speak with the other person directly. Most concerns relate to unethical behavior of a compliance nature. A person making a complaint will not be penalized nor suffer retribution for submitting a concern. Please document your concern in a factual, professional manner.

Though there is not a set time limit for reporting your concerns, we recommend that you submit a report as soon as possible after a situation arises or after unsuccessful attempts to resolve the issue.

When you submit this report, Student Affairs and the Dean of Students receive the report. The Dean of Students will review the report and determine the best course of action. The Dean may meet with the individual submitting the report and/or the individual about whom the report is made. The Dean may refer the report to another appropriate party or department, such as a school’s dean/associate dean/assistant dean, program director, or human resources.

We endeavor to respond to your report as soon as possible to notify you of receipt of the report. If you wish, we can follow up with you, though due to confidentiality, we may be unable to share many details about any actions taken.

Submissions are sent to Student Affairs, and directed to the appropriate individual and/or department.
