Confidential Reporting

The purpose of the hotline is to allow employees who are not comfortable raising issues directly with the person(s) involved, report concerns.

The purpose of this form is to allow for confidential reporting of any type (e.g., sexual harassment, crime, complaints of accreditation standards, ethics violations, professionalism, or other). The confidential report will be forwarded to the appropriate administrator for review. Completing the confidential report form will not necessarily initiate any law enforcement, judicial, or administrative action against a perpetrator.

If you wish to receive feedback regarding your report, you must include contact information. Those who receive this report will take steps to keep your identity confidential and guard against any adverse action for filing the report.
