Healthy & Engaged

We value healthy and engaged communities, patients, healthcare providers, and partners, particularly in rural and medically underserved areas.

Person-Centered

We value person-centered care that encourages shared decision-making and is delivered with an appreciation of diverse cultures, without bias or judgment.

Sustainable

We value a sustainable model of collaborative healthcare practice that supports safety and quality and provides improved outcomes for those served, particularly in rural and medically underserved areas.

Equity

We value healthcare equity and the removal of barriers that prevent access and collaboration in healthcare.
