Office of Compliance, Ethics, and Integrity Services

The PNWU Office of Compliance, Ethics, and Integrity Services provides oversight, resources, consultation, and training for the PNWU Compliance, Ethics, and Integrity Program (“Program”) to fulfill the University’s commitment to maintaining integrity in all aspects of University operations and compliance with all applicable laws, regulations, and industry standards. The Program is intended to promote a culture of ethical behavior throughout the University; communicate expectations set by the Board of Trustees regarding the ethical and legal responsibilities of all employees; promote the prevention, detection, and resolution of instances of noncompliance; and provide employees with the means to report or address concerns regarding unethical or unlawful behavior.
Compliance, Ethics, and Integrity (CEI) Program Overview

Confidential Reporting of Suspected Wrongful Conduct
“Members of the University community have an affirmative duty to disclose and seek guidance if they believe that a member of the University community or person associated with or doing business with PNWU is engaging in wrongful conduct.” –Reporting Suspected Wrongful Conduct Policy

The form linked below is used to report known or suspected wrongful conduct to the Office of Compliance, Ethics, and Integrity Services (CEIS). Wrongful conduct is any action or omission taken by or within the University that is illegal, fraudulent, or in violation of University policies, including the Code of Conduct, or procedures. Wrongful conduct includes, but is not limited to, violations of law including fraud; unauthorized use of University property or resources; fraudulent or dishonest financial reporting, bribery, or kickbacks; and research or other academic fraud.

For employees, students, and volunteers, reporting of this kind should normally be made initially through standard management channels, beginning with your immediate supervisor, instructor, or advisor, or the appropriate committee, office, or department. If for any reason it is not appropriate for you to report known or suspected wrongful conduct to your immediate supervisor or one of the aforementioned (e.g., the suspected violation is by your supervisor), you may go to a higher level of management. If you elect to complete this form, you will be asked to explain prior attempts to resolve the issue or the reason(s) why such attempts are not appropriate.

If unsure whether you should complete this form, you are encouraged to contact CEIS (compliance-ethics@pnwu.edu).

Please provide as much information as possible to ensure timeliness and thoroughness in addressing your submission. The form allows for uploading of documents. By providing your contact information, someone from CEIS can contact you if additional input or clarification is necessary and to provide appropriate feedback/updates. CEIS will do its best to protect your anonymity and keep your information confidential.

We expect that your report is made in good faith to address legitimate issues needing correction, or to otherwise provide reliable information. If you are reporting known or suspected wrongful conduct in good faith, it is protected under the PNWU Reporting Suspected Wrongful Conduct Policy and PNWU Whistleblower Policy, which prohibit retaliation against anyone disclosing a violation or noncompliance with laws, rules, or regulations.

NOTE:
If your report or issue is related to Title IX (see Title IX Sexual Harassment, Including Sexual Assault, Dating violence, Stalking, and Retaliation Policy), please use the Title IX Report Form.

If your report of issue is related to a bias incident, please visit the Office of Diversity, Equity, and Inclusion web page and/or use the Bias Incident Report Form.

If you are reporting complaints related to accreditation or student complaints of program integrity, please see links on the Accreditation page at PNWU.edu.

This form is not for appealing grades, accommodation decisions, dismissal, etc. Please see the applicable Student Handbook or visit the applicable page on PNWU.edu or MyPNWU for those procedures.
